Six departments form the backbone of your hospital: the Emergency Room handles walk-in trauma cases, the ICU manages critical patients requiring continuous monitoring, the Operating Theatre performs complex surgical procedures, the Diagnostics Laboratory processes blood work and imaging, the Pharmacy synthesizes and dispenses medication, and the Research Center drives breakthrough medical discoveries with the highest gold yield of any department.

Each department can be leveled up for higher payouts and eventually automated with staff — doctors, nurses, and AI systems that work without player input. The upgrade shop offers eight permanent improvements split between click power (Stethoscope, Expert Surgeon Hands, AI Diagnosis Assistant, Nano Medicine Protocol) and automation efficiency (Night Shift Staff, Hospital Management AI, Express Care Protocol, Quantum Research Engine).

The prestige system rewards long-term play with compounding multipliers. Each prestige reset resets your progress but permanently increases all gold income, enabling exponentially faster progression in subsequent runs.
